Fact Sheet
1. Trip date & place: December 20-29, 2009 - Parrita, Costa Rica.
2. Purpose: Build a church that would function as a community center; bring Christmas gifts to poor children; provide medical care and share Christ with the locals.
3. Main Sponsor: Bloomington Seventh-day Adventist Church.
4. What is the first step?
Submit an application with a $100 non-refundable commitment deposit that will be used to reserve your ticket and will apply towards total cost. Turn ASAP if you want to qualify for the early bird.
5. Funds: Each participant is responsible for raising his/her own funds.
6. Cost per person: Price includes all expenses in Costa Rica, international round-trip airfare ticket from Indianapolis, inland transportation, travel insurance, food, lodging, mission T-shirt and one-day tour:
- $ 1,100 - Early Bird (if paid by October 4, 2009)
- $ 1,200- Regular Registration (if paid after October 4, 2009)

7. Who will arrange details in Costa Rica?
Pr. Fernando Ortiz along with local SDA Conference administration and district pastors will be responsible and personally involved in the project, making sure you are safe, comfortable and achieving success in our mission.
8. Passport requirements: 2 (2×2) passport pictures (can be taken at Walgreens or Wal-Mart. No vending machine pictures accepted); Birth certificate with seal (no photocopies); $97 Fee. Bring these documents to the local post office. It will take from 4-6 weeks to process. 9. Vaccination. No vaccination required. We recommend Hepatitis A. (Administered free to minors in some counties or city health agencies). Check the Center for Decease control webpage for updates on immunizations: http://wwwn.cdc.gov/travel/
10. Luggage allowance: 2 suitcases not exceeding 50 lbs each. One piece is for your personal items. The other is reserved for Mission trip supplies, crafts, etc.
